CLINIC REGISTRATION

You must register to attend a clinic in advance of the clinic date. We accept registrations up to 24 hours prior to the published clinic date, but recommend you register a minimum of one week ahead. There are two methods of registration:

2. By completing and submitting the registratIon form below

CLINIC FEE: $69.00

 PAY CASH OR MONEY ORDER THE DAY OF THE CLASS OR PAY IN ADVANCE BY PAYPAL/CREDIT CARD

Items you must bring to class:

1. Government issued ID (Driver's License or ID Card)
2. Payment for the clinic or your online payment receipt
3. Notice from the DMV or Court if appropriate
